Approval LaPaz COUNTY HEALTH DEPARTMENT if connecting to sewage disposal system (septic).

8 ½ X11 Plot Plan showing location of septic tank & leach lines, gas lines, water and sewer lines, overhead power lines, paved and unpaved roads, and driveways, fences, easements, other structures, washes, ditches and streambeds. Electrical Meter locations must also be shown (36” X 30” is required in front of meter).

Copy of original sales contract (permit required for all accessory structures not included on contract).

Permit required for all ground set cooling/heating units.

Permit required for gas line install.

Tie Downs are required.

Ground set installs with retaining walls, plans to be submitted to State for approval.

Must be onsite at time of inspection.

Slabs must meet States design requirements (4000 psi.)

Assessors Parcel Number

Homeowners Association Approval (if applicable)

RV or Mobile Home Park Approval (if applicable)

Floodplain Permit (if applicable)

GENERAL INFORMATION

Approved plans and manufacturers installation manual on site for all requested inspections

Permit card must be posted on job site at all times.
